  Please make sure to notify us of any name, address, phone # changes, etc when you place your order..
  Please note a ship date, cancel date and PO number if applicable.
  
  You will receive back your detailed order copy complete with pictures, pricing, UPC information, etc.

    (If you are an old KSA customer and have your KSA account number, please send us that information so we can update our records!)

  Once we receive your information, you can get pricing information for all themes and categories shown on our web site as described above.
  
  Then, just send us a list of items you want & quantities you want. Let us know your ship and cancel date and PO # if you have one.
  
  Please fill out a credit card form and fax or email it back to us. You can click here for a downloadable credit card form. If you want to apply for credit terms with Kurt Adler, please send us your credit information by email or fax. You can also request Kurt Adler's credit form by email to us.
  
  We will email back a final order copy and place your order.
